☐ Attach petition / notification documentation if required
☐ Attach site map / street diagram showing barricade placement
☐ Include payment (check/card) if fee applies
☐ Save confirmation / receipt from submission

🚧 Phase 4: Logistics (1–2 Weeks Before)

☐ Reserve Type-III barricades from a private rental vendor (need 4–6 for a through-street)
☐ Confirm barricade delivery time (must arrive before your start time)
☐ Confirm permit approval received (call permit office if not received 5 days before)
☐ Note your city's noise ordinance cutoff time: ________ PM
☐ Confirm any bounce house/tent vendor has their own liability insurance
☐ Send reminder flyer to all block households 3–5 days before event

🎉 Phase 5: Day Of

☐ Print permit and place in waterproof sleeve on a barricade
☐ Place barricades at both ends of closure at approved start time
☐ Verify emergency vehicles can still access every home on the block
☐ Designate a noise monitor — set phone alarm 30 min before ordinance cutoff
☐ At event end: music off, barricades removed, street cleared
☐ Remove all equipment, trash, and signage from the street
